The booking form, questions and waivers

What the booking form asks, adding your own questions, and attaching an e-signed waiver or an agreement checkbox.

What every form asks

Name and email are always asked. Preferred event contact is a switch per event, on by default: the client picks phone, email or WhatsApp and enters it, and it is shared with the host for messages about that session — every host sees it, whatever the team's contact-details permission says, because the client chose to share it. A phone-call event always asks for the number. For group bookings the client can name the other participants.

Your own questions

In the event's Booking form section, add questions with a label and an answer type: one line, multiple lines, a dropdown, a checkbox list, an agreement checkbox (with an optional link to what they agree to), or a phone number. Each question's Required switch decides whether clients must answer it (or tick it, for an agreement checkbox) before they can book; a new question starts with the choice you made when adding it. Change a question's Answer type in its row at any time; click the question to edit its text, its options or an agreement's link. A dropdown or a checkbox list needs at least two options, and answers already given stay as they were. Reorder with the arrows, or delete a question (asked twice) to take it off the form — one that already has answers keeps them on their bookings. A question marked Not on your booking form is kept but not asked: Put it back on the form asks it again, or delete it.

Where answers go

Every answer stays on its booking: Form answers on the booking's row in the Bookings tab opens all of them in one window, in the form's order, with a link to the person's contact page. The latest answer to each question also shows on that contact page.

Each question (except an agreement checkbox) has a Contact field that also keeps its answer on the contact: None (the default), Referred by (filled in when none is recorded), A label (this question's earlier label is replaced), A note (added when the answer changes), or one of your own columns (the latest answer). New column… adds a text column to the Contacts table right there. A contact field applies to answers given after you choose it. In the Form answers window, an answer whose question has a contact field is marked with it, such as "Contact field: Referred by". "How did you hear about us?" kept as Referred by, or "Experience level" kept as a label, are two ways to use it.

Waivers and forms

Documents live in Settings → Forms & waivers: a name, the text, and a signing method:

  • Typed e-signature — the client types their name in a signing window before booking; the signed name, date and document version are kept on the booking.
  • Agreement checkbox — one checkbox on the form.

Each document counts the events using it. Apply to all events and Remove from all change every event at once; a new event starts with your document when you have exactly one. Copy link gives a public page of the text you can send ahead, where anyone can read it and sign it.

A client on an event's waitlist keeps the version they signed when they joined, and the booking made for them carries it; a new version of the same document never takes them off. A different document does: when an event starts asking for another agreement, or for one where they signed none, their place ends and they are emailed to join again. Apply to all events says first how many people waiting on those events that takes off (see The waitlist for a time). Removing a document takes nobody off.

The person who books agrees for their whole group. Each of the others can also sign for themselves: the confirmation page and the confirmation email carry the document's link when a booking has more than one participant, and the booking page's signing window has Copy link and Email it. Whoever opens the link gives their name and email and agrees the way the document asks (a typed name or the checkbox). Their signature, the date and the document version appear in Contacts next to the clients who booked, so you can see who has signed before the session.

Editing a document creates a new version. Bookings keep the text they were signed against, so an old signature never changes meaning.

Per event

The event's Booking form section picks which document applies, or none. The event card shows "No waiver" when nothing is attached.

Longer forms after booking

For a longer set of questions — a health history or a readiness check, asked before a first session rather than at every booking — use an intake form instead of more booking questions: clients fill it in from a link after they book, and it lives under the waivers in Settings → Forms & waivers (see Intake forms).
